NOTE: Requested dates are subject to approval by monterey media inc. in order to avoid booking conflicts.

All Terms and Conditions listed are applicable in this contract. Exhibitors are responsible for seeing that those involved with their Event adhere to these regulations.
 1.I agree that I can only exhibit the film on the date specified above.
 2.I agree that I am responsible for all costs associated with the exhibition of this film.
 3.I commit to pay $125 to screen the film if less than 25 people attend, and $250 if 25 or more people attend. Payment is due 30 days prior to the event.
 4.I agree to return the projection element within one week of my showing(s) in its original condition.
 5.I will submit the dates of all showings to monterey media, inc. If any of the dates change from what I originally submitted or I add additional showings, I agree to notify the monterey media, inc. office.
 6.I agree to check my venue location information for accuracy on the films website, and to request necessary changes by contacting the event coordinator by email.
 7.I do not have rights to make any copies of the film whatsoever.
 8.I understand that no audio or video recordings of the movie will be allowed at any of my showings and I agree to take measures to enforce this.
 9.I can expect to receive the projection element approximately two weeks prior to my showing. I agree to test this element upon receipt.
 10.I understand that monterey media, inc. will be responsible for shipping expenses of the exhibition element to the event and that I will be responsible for all shipping expenses of the exhibition element from the event.
 11.I understand that monterey media, inc. is the exclusive US distribution company for the film.
 12.Payment for the Film: Payment can be made via Check, Money Order, or Credit Card. Please make Checks or Money Orders payable to: monterey media, inc., 566 St. Charles Drive, Thousand Oaks, CA 91360.
 I AgreeI understand and agree that by submitting this contract and agreeing to the above terms and conditions, this becomes a valid agreement between monterey media, inc. and myself/organization.
